AFFILIATE MEMBERSHIP APPLICATION

This membership is for a Licensed Practical Nurse or other health and safety professional practicing within Occupational Health or a Registered Nurse enrolled in an OHN training program but not yet graduated.  Affiliate Members are eligible to serve on a Committee and attend the AGM in a non-voting capacity. 

Please be sure to upload a copy of your professional credentials.  If you are a Registered Nurse enrolled in an OHN training program, but not yet graduated, please provide a copy of a current transcript.  Your membership application cannot be processed without this document.
